Starting October 15, 2014, the Division of Sponsored Programs and Division of Research Compliance will require investigators to use the new Public Health Services (PHS) Disclosure of Significant Financial Interests form and the National Science Foundation (NSF) Disclosure of Significant Financial Interests form which can be found at the following URL links:
PHS – http://research.ufl.edu/compliance/pdf/PHS_DSR5_rev1.pdf
NSF – http://research.ufl.edu/compliance/pdf/NSF1_DSR5_rev2.1.pdf
Please note the following changes to the PHS Disclosure of Significant Financial Interest (SFI) form:
- The inclusion of a link to those organizations that adopt the PHS Conflict of Interest policy
- The inclusion of a question (located at the bottom of page 2) that addresses non-federal conflicts of interest
Again please note that starting October 15, 2014 these forms will be the only ones accepted for use and old versions of the form will be returned and could cause a delay in submission of your grant proposals.
Should you have any questions contact Mike Scian at scianmp@ufl.edu and if you are in the college of medicine Gary Wimsett at gwimsett@ufl.edu
